Refer A Concern or Incident

Use this form to refer students to CCA’s Student Advocacy Services for support, resources and access. Students may be referred for concerns including:

• Life Circumstances (grief and loss, major life events)
• Navigating institutional processes
• Basic Needs (food, housing, childcare, transportation, emergency financial needs)
• Emotional Support, Mental Health, and Well-Being (emotional distress, mental health concerns, need for additional support)
• Concerning Behaviors (changes in behavior, safety or well-being concerns)
• Disability Accommodations and Access (questions about accommodations, guidance through the application process)

Use this form to file a College Complaint if you believe you’ve experienced unfair or improper treatment. This form should not be used for: 

• ADA Discrimination 
• Academic Appeals 
• Students who wish to contest a final grade should first speak with their instructor, then, if needed, the department chair or program director. If unresolved, they may submit a written appeal to the division dean. See full guideline CCA ACS 2.20 for more information.  
• Student Conduct Appeals
• Students who wish to contest the outcome of their conduct meeting may submit a written appeal. See full guideline CCA STS 7.04 for more information.
• Title VII Discrimination 
• Title IX (Sexual Misconduct/Gender Discrimination) 

Use this form to report incidents of discrimination, harassment, or retaliation involving CCA students, faculty, instructors and/or staff to the Coordinator of Title IX & Equal Opportunity. This includes sexual misconduct and other behaviors that violate Title IX:

• Dating and domestic violence
• Gender-based discrimination
• Sexual harassment
• Sexual assault
• Stalking

Use this form to refer students who may be in violation of the Academic Integrity Policy  

Use this form to refer students who may be in violation of the Student Code of Conduct.

File a complaint about any Colorado institution of higher education.

Use this form as an anonymous way to report anything that threatens your safety or the safety of others. Please note this is not an emergency reporting system. Dial 911 for emergencies.
